Death

You make a Death Saving Throw when your Vitality reaches 0, or when you take damage when your Vitality is 0.

To make a Death Saving Throw, roll a d20 saving throw with a base Difficulty Class of 6.

For every successful Death Save, the Death Save Difficulty Class increases by 2, until the next Rest.

On a successful saving throw, you survive. On a failure, your character dies.

If you take an amount of damage equal to 2 times your maximum vitality from a single source of damage, you die.

To increase the difficulty and the chance for a character to die, your GM may increase the default Death DC, or lower it to decrease the difficulty.
