Symptoms: The afflicted begins hearing whispers that repeat their thoughts back to them, sometimes distorting the words into sinister meanings. As the disease progresses, the whispers grow louder and more frequent, making it difficult to concentrate or sleep.
Stage 1: The afflicted hears faint whispers whenever they’re alone or in silence. They take a -1 status penalty to Perception checks due to distraction. (1 day)
Stage 2: The whispers become louder, echoing the afflicted’s thoughts and causing them to doubt their actions. They suffer a -2 status penalty to Will saves and must succeed at a DC 20 Will save before attempting a complex task, such as casting a spell or making a Crafting check, or become stupefied 1 for 1 hour. (1 day)
Stage 3: The whispers transform into full-blown voices that argue with the afflicted, causing them to struggle to distinguish reality from hallucinations. They gain the confused condition for 1d4 rounds whenever they attempt a complex task, and suffer from constant sleep deprivation, imposing a -2 status penalty to all checks and saves. (1 day)
Cure: 3 consecutive saves; alternatively, psychic healing spells like calm emotions or remove fear (4th-level or higher) can suppress the voices for 24 hours, granting the afflicted a +2 circumstance bonus to their next save against the disease.
Special: The disease thrives in areas rich with psychic energy or where traumatic events have occurred, making it common in haunted locations or cursed ruins.
