The air is incredibly thin. Creatures who aren’t acclimated must attempt a Fortitude save each hour they breathe thin air, or become fatigued (and slowed 1 on a critical failure). The effect persists until the creature gets 8 hours of rest. Acclimating means a creature has spent at least 1 week living in the environment with daily exposure.
