P = Planet of birth
DDDDDD = Date of birth
The Calendar Tool will give you the proper date in its compact written form, and as such, you can follow these skeleton examples to properly create your PKZ by hand. Moreover, the tool itself has an "Info" tab which has more information on PKZs, and a native PKZ generator that you can use to automatically create your PKZ.
The above generic skeleton does not take into account different forms of PKZs by their respective planets — this is is a visual guide so that you may understand what a PKZ is.
Below is a list of all planets (and a skeleton PKZ to match) to help guide you if you wish to write the PKZ yourself. Each PKZ will have different styles for denoting the planet of birth.
For your character's name, you are allowed to use European and East-Asian first and lastnames, but not Romance language names (French, English, Spanish, Latin, Romanian, etc.).

Note: Rotfront has negative Seasons. If you are to handcraft your PKZ, you will add paranthesis to denote the negative from the dash between the planet of birth.

Note: Both Heimat and Leng share the same format, but are different date-wise because of their orbital characteristics. Heimat & Leng's Cycles are made up of hexadecimal numbers, which go from 0-F. The letters past 9 are capitalised.
